(“Company”) approved the fourth amendment (“Amendment No.”) to the 2017 Equity Incentive Plan of Fuse Medical, Inc., dated April5, 2017 (the “Plan”), filed with the Securities and Exchange Commission on April6, 2017, as Exhibit 99.2 to its Current Report on Form 8-K, and incorporated by reference. Amendment No.4, filed as Exhibit 10.1 to this Current Report on Form 8-K, increased the number of shares of common stock of the Company, par value $0.01 per share (“Common Stock”), authorized for issuance under the Plan from 6,000,000 shares of Common Stock to 8,000,000 shares of Common Stock.

Fuse Medical, Inc., formerly Golf Rounds.com, Inc., is engaged in marketing, distributing and selling internal fixation, bone materials, biologics, tissues, surgical and other related surgical products.
